AGLMH Electronic Award Nomination Form

Before submitting your nomination, be sure to review the nomination guidelines for the awards. You may wish to write your nomination comments separately and paste it into the comments field when you submit it. The form will email your nomination to the Awards Committee Chairperson and will send a confirmation copy of the email to the submitter. Questions or problems with this form can be addressed to the webmaster. Questions or comments on the awards, nomination or selection process can be addressed to the Awards Committee Chairperson.
